Practice Lists are currenly open to patients living within our practice area.

You can register by completing a form available from reception. Proof of address and identity are required before registration process can take place. Details of acceptable docuements are available from reception.

If you have a medical card with another GP you will be asked to sign a transfer form to have your number transferred to this practice. Also, you will need to sign a letter of authorisation to obtain your medical records from your previous GP.

You may be asked to underake a new patient health check with your new GP or Practice Nurse.
