Repeat Pescription Order Forms available in our surgery to the left of the reception desk just inside the main door. 

Order forms for repeat prescriptions can be faxed to 01 8530891, emailed to meds@fairviewmedical.ie or delivered by hand. Please fully complete the form and it can be posted in the letter box to the left of the reception desks.

Due to the large volume of repeat prescriptions we would appreciate 48hour notice.

If you wish your prescription to be posted to you, please supply a stamped addressed envelope otherwise the prescription can be collected at the reception desk following 48 hours from order.

Please note as with all contacts at reception is is necessary to take a ticket so that we can deal with all patients in an orderly and efficient manner. Also, it would be very helpful if you call outside clinic times e.g. 12 mid to 1:30pm when the surgery is less busy and therefore there will be less waiting time to collect your prescription.
